Commercial licence forms

This page provides forms related to commercial fishing activities in Western Australia.

To register for Fish Eye Online Services and nominate online user roles, download and complete forms ET-1 and ET-2 from the list below. Online applications are not currently available. For more information visit Fish Eye.

Please note the following conditions related to our forms:

• As our forms may be amended at any time, please download and print them as required. Form T3 (Application for the Temporary Transfer of Part of an Entitlement) can be completed online. See Fish Eye Online Services for more information.
• Once downloaded, the text/format of the form should not be modified as this could make the form invalid.
• When completing our forms, please carefully read the instructions provided. Failure to complete all sections of the form could make the application invalid and it may be returned to you.
• Stamp duty may be payable with regard to forms T1, T2, T3 and V2. The Office of State Revenue can provide further details about stamp duty and the requirements to be met before the Director General of the Department of Fisheries can accept these application forms.

Online services

• Apply for a Commercial Fishing Licence online – you will need your credit card details to pay the application fee.
• Renew your Commercial Fishing Licence online – you will need your application for commercial fishing licence renewal notice and credit card details to pay your renewal fees.
• Transfer temporary entitlement online - if you are a Fish Eye Online Operator you can transfer your temporary entitlement online. You will need your credit card details to pay transfer fees. If you are not a Fish Eye Online Operator and want to transfer temporary entitlement, you can download the T3 form - Application for the Temporary Transfer of Part of an Entitlement - on this page.
• Entitlement Exchange advertising board – if you are a licence holder you can advertise the temporary or permanent transfer of an entitlement, and permanent transfer of an Authorisation. This advertising board can be viewed by all visitors to this site.

Application for the Grant of a Commercial Fishing Licence Used by the applicant to apply for a commercial fishing licence to work as a deckhand or skipper on a commercial fishing boat.	see Schedule 1 of FRMR​​
Application For The Release Of Information Used by the applicant to apply for catch/operational history or entitlement use information held by the Department relating to an authorisation.
Fish Eye Online Services Registration Form (ET-1)   Used to register individuals for secure access to Fish Eye Online Services.
Fish Eye Online Services Nomination Form (ET-2)   Used by Authorisation or Certificate Holders to nominate individuals into specific online user roles and act on their behalf using Fish Eye Online Services.	​
Fish Eye On​line Services - Application to revoke the approval of ​a Nominated Person Form (ET-3) Used to revoke the approval for a nominated person to act on behalf of the licence holder or the holder of a registered receiver certificate.​	​
​Enduring Consent of Security Holder to the Temporary Transfer of Entitlement​ This application form is to be used by the security interest holder to provide industry consent to the temporary transfer of entitlement.	​
​Security Interest Holder Consent Form This form is to be used to give consent to an application for the Temporary Transfer of unit entitlements.	​
​Notice of Revocation of Enduring Consent of Security Holder This application form is to be used by the security interest holder to revoke industry consent to the temporary transfer of entitlement.	​
Application for a Permit to Construct, Establish or Modify a Fish Processing Place Used by the applicant to apply for a permit to construct, establish or modify a fish processing place on land or at sea, prior to a Fish Processors Licence being applied for and granted.	​see Schedule 1 of FRMR​
Application for a Fish Processor's Licence Used by the applicant to apply for a Fish Processor's Licence to process fish.
Notification of Intent to Become a Registered Receiver of Rock Lobster - West Coast Rock Lobster Managed Fishery Used by the applicant to register with the Department of Fisheries as a receiver of rock lobster caught within the West Coast Rock Lobster Managed Fishery.
Application for a Copy of an Entry, or Extract from the Register   Used to obtain information on licence details.	see Schedule 1 of FRMR​
Application for Notation of Security Interest Holder on the Register Used by the authorisation holder to make a security interest notation on the Register	see Schedule 1 of FRMR​
