These forms are to be used in appling for an Aquaculture licence and are to be lodged with the Executive Director of Fisheries WA at the address indicated, together with any additional information required in support of the application (additional information requirements for species are available for download below, this includes the main application form and the information sheets as a complete kit for each species).
Aquaculture is a developing industry in Western Australia and licensing policies for specific aquaculture species are under constant review. Applicants should therefore ensure that the latest information is obtained from the Department of Fisheries on current and proposed licensing policies for the fish species that are the subject of the aquaculture proposal.
If the aquaculture proposal affects land other than private land - eg. Crown Land or waters (including coastal waters), a more detailed set of guidelines and an additional information sheet is available.
There are Additional Information Sheets for the following species. The relevant sheet(s) must be completed and submitted with this Application Form.

An Aquaculture licence is issued subject to the conditions set out in Regulation 69 of the Fish Resources Management Regulations 1995 and as may be specified in the licence by the Executive Director (who may vary or cancel a condition by notice in writing).
Where facsimile applications are submitted, the original application form must be submitted to the Department of Fisheries.
The prescribed application fee for applications relating to non-freehold land or freehold land are available here. These fees must accompany this application.

Applicants should be aware that the details disclosed in this Application may be recorded on the Licence Register and be available for public search.
